Option 1: Guided Conversations: Individuals
Highly skilled, certified individuals are available to meet with you (and those who matter most to you) to guide an advance care planning conversation. This conversation will help you explore your experiences, goals and values that can help you make decisions regarding your future healthcare. Perhaps you have a book club, faith group or community service organization that meets regularly who would like to learn more about advance care planning. Honoring Choices® Idaho will help connect you and your group with a certified advance care planning facilitator to meet with your group to explore experiences, goals and values that can help people make decisions regarding future healthcare.
